Records Addition Form

Every record housed in Central Storage must have a record series number.  If the record does not have a series number in the General Schedule (GRDS) or in the Division Schedule (85-09) a new series number should be requested.

To request a new record series number for your Division Schedule, please fill out the following form and send to at the Service Center. A sample Record Inventory Form is also provided.
