Candidates must be 18 years old.
Candidate must complete an approved training program.
Testing Fee: $150
Required Documentation:
- Original approval letter from the Oklahoma Board of Nursing
- Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
