Certified Nurse Aide Level 2 & 3
Candidates must have a current nurse aide certification in long-term care.
Candidate must be recommended for and complete an approved training program.
Candidates must have six months work experience as a Long Term Care CNA.
Testing Fee: No fee charged to student
Required Documentation:
- Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
- Original proof of Social Security number or Taxpayer Identification number (photocopies will not be accepted)
- Verification of current Oklahoma long-term care nurse aide certification
Students must test at the tech center where they received training.
