Emergency Medical Responder
Testing Prerequisites:
Candidate must complete an approved training program
Testing Fee: $25
Required Documentation:
- Original ODH 681, EMR Training & Testing Verification Form
- Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
- Original proof of Social Security number or Taxpayer Identification number (photocopies will not be accepted)
NOTE: In order to be listed on the Oklahoma EMS registry, candidates must mail two documents to the Oklahoma State Department of Health EMS Division: (1) original completed ODH 681 Training & Testing Verification Form and (2) $10.00 fee. Forms will not be accepted electronically.
