Medication Aide

Candidates must have a current nurse aide certification in long-term care, home health, and/or developmentally disabled care.

Candidate must complete an approved training program.

Candidate must attest that he/she meets all of the following requirements:

  • Is at age 18 or older
  • Has a high school or general equivalency diploma
  • Has a current Oklahoma nurse aide certification with no abuse notations
  • Has experience working as a certified nurse aide for six months
  • Has the physical and mental capability to safely perform duties

Testing Fee: Contact HCP Testing Center

Required Documentation:

  • Original Training Verification Form signed by the training program supervisor
  • Medication Pass Worksheet documenting accurate medication passes to 20 individuals
  • Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
  • Original proof of Social Security number or Taxpayer Identification number (photocopies will not be accepted)
