Testing Prerequisites:
Candidate must complete an approved training/apprenticeship program and apply for certification/approval to test
Testing Fee: $200
Required Documentation:
- Original approval letter from Oklahoma State Department of Health
- Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
