Advanced Certified Medication Aide

Candidates must have a current nurse aide certification in long-term care, home health, and/or developmentally disabled care.

Candidate must complete an approved training program.

Testing Fee:  Contact HCP Testing Center

Required Documentation:

• Original OSDH Form 504 documenting successful completion of training
• Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
• Social Security Card or Taxpayer Identification Card (Note:  Must present actual card - the test center will not accept photocopies or substitutes.)
• Current Oklahoma nurse aide certification card
• Current Oklahoma certified medication aide certification card

In order for the advanced designation to be noted on the Oklahoma Nurse Aide Registry, the candidate must send OSDH Form 504, HCP Testing Verification Form, and the required fee to the Oklahoma State Department of Health.
