Certified Nurse Aide Level 2
Candidates must have a current nurse aide certification in long-term care or combination.Candidate must complete an approved training program.
Testing Fee: No fee charged to student
Required Documentation:
- Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
- Social Security Card or Taxpayer Identification Card (Note: Must present actual card - the test center will not accept photocopies or substitutes.)
- Current Oklahoma nurse aide certification card
Students must test at the tech center where they received training.
