Emergency Medical Responder

Testing Prerequisites:

Candidate must complete an approved training program

Testing Fee:  $25

Required Documentation:

• Original ODH 681, EMR Training & Testing Verification Form
• Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)
• Social Security Card

NOTE:  Upon completion of training and testing, candidate must mail the original ODH 681 to the Oklahoma State Department of Health EMS Division.  Forms will not be accepted electronically.
