Home Care Administrator

The Health Certification Project offers the Oklahoma Home Care Administrator Preparedness Assessment for the Oklahoma State Department of Health

Testing Fee: $100.00

Required Documentation

  • Original certificate/letter of completion from training program OR original testing authorization letter from OSDH OR provisional certificate issued by OSDH

  • Valid, current photo identification issued by government entity within the U.S. (i.e. passport, driver's license, school-issued ID)

  • Social Security Card or Taxpayer Identification Card (Note:  Must present actual card - the test center will not accept photocopies or substitutes.)
